Almost all actions used to have a chance of a critical failure. When a critical failure occurs the action has no effect at all, but the AP is still used. Originally, most actions critical failures were fatal (Consumes the AP cost of spell/weapon and ends turn regardless of extra AP/MP), now its only fatal for weapons. Spells that are once-per-turn can be recast if not fatal.
